In Network

I am an in-network provider with the following insurance companies:

  • Regence/BCBS

  • Pacific Source

  • Providence

  • Moda/ODS (Note: I am not in network with Moda Beacon)

  • Aetna

I am able to bill all of the above insurances for both telehealth and in person visits.

If you wish to use insurance for your visit, please call your insurance carrier prior to your appointment to confirm your plan benefits and to verify that I am an in-network provider with your specific plan. Please remember that ultimately your insurance benefits are a contract between yourself and your insurance company and that it is your responsibility to be aware of your benefits.

Out of Network

If you do not see your insurance carrier in the above list, I am happy to provide a Superbill for you to submit to your plan to use your out-of-network benefits.

Cash Pricing:

  • New Patient Visit (60 Minutes): $325

  • Return Patient Visit (with or without Acupuncture/Bodywork) (60 Minutes): $175

  • Return Patient Brief Telehealth Check-In (30 Minutes): $125

  • Blood Draws: $30

  • B12 Injections: $15 (plus cost of nutrients)

  • Intrauterine Insemination (fresh sperm): $550-650

  • Intrauterine Insemination (frozen sperm): $350-450

Please note I am not a participating provider with Medicaid or Medicare. Please inform our front desk prior to your appointment if you use either of these insurance options.
